Well at Home Befriending/Mentoring is a service for Newham residents (aged 50+) who have recently been discharged from hospital/community health services or are socially isolated who would like short-term practical and emotional support to re-establish confidence and return to independent everyday activities.
How does it work?
Referrals normally will come from health or social care statutory agencies.
After a referral is received, the Service Manager will make an appointment to do an assessment at the Service User's home. The suitability of the Service User is assessed and if suitable, goals are discussed and agreed for the service period. The Service User may also be referred to other available services if required.
The Service User is then matched with a befriending volunteer who is then introduced by the Service Manager and the weekly home visits are scheduled for the following six weeks.
At the end of the six week period, the Serivce Manage will carry out a closing assessment and any additional services can be discussed and agreed with the Service User. Telephone befriending is available on completion of the service for a regular friendly chat and catch-up.
What are its goals?
The primary aim of the service is achieving the Service User's goals during the befriending/mentoring period.
With the support of a volunteer befriender/mentor, the Service User's may, for instance:
a) want to become involved in local social or leisure activities;
b) do some local shopping together;
c) go together to collect their pension or do some banking;
d) go for local walks;
e) practice mobility around the house and garden.
And of course have a chat and a cup of tea!
There is no charge for this service.

What is NOT provided
1) housekeeping and household tasks;
2) administration or monitoring of medication;
3) personal care.
